Covid-19 Questions
Patients, please reschedule if you meet the following criteria
- Have you experienced any of these symptoms in the last 10 days – Fever, Chills, SOB, Cough, Loss of Taste/Smell, Fatigue, Muscle or Body Aches, Nausea/Vomiting?
- Have you Tested, Been Ask to Test, or Quarantine in the last 14 days?
- Do you Live with Someone, have Been Around Anyone with Covid, or Who had Symptoms in the last 14 days?
- Have you been in a Crowd greater than 50 without Social Distancing?
- Have you been in a Situation with Close Contact where you could not Socially Distance in the last 14 days?
(Restaurants, Church, or other places where distance rules are set in place DO NOT COUNT)
- Have you Traveled OUTSIDE THE US in the last 14 days?
